Step 1: How to Request a New Patient Appointment

- Provider Referral: Have your physician send us your referral
- Online Appointment Request: Click on Patient Portal and Create a New Account
Once your request is received, it is reviewed by our scheduling team.
Step 2: New Patient Scheduling Nurse Coordinator Contact
A New Patient Scheduling Nurse Coordinator will contact you to:
- Review the reason for your visit
- Determine the urgency of care
- Match you with the most appropriate provider
- Identify available appointment times
This personalized process helps ensure you receive the right care at the right time.
Step 3: Patient Portal Registration for SWC
All new patients are required to register through the SWC Patient Portal. Portal registration allows us to securely collect your health information and establish your medical record prior to your visit.
The patient portal also provides easy access to:
- Appointment details
- Secure messaging with your care team
- Prescriptions and refills
- Test results
- Treatment plans
Step 4: Choosing a Women’s Health Provider at SWC
Patients may choose a provider for ongoing women’s healthcare. Provider availability varies, and some providers may have longer wait times than others.
