Mesh-Free Pelvic Prolapse Repair
Pelvic Organ Prolapse repair without permanent mesh is our most common surgical treatment for prolapse of the uterus, bladder or vaginal walls. This is done with laparoscopy or a vaginal approach. Poor vaginal support of the bladder is a cystocele. The vaginal wall can be strengthened surgically with sutures to help support a "dropping" bladder. When the floor of the vaginal is weak, the rectum bulges against the posterior vagina. This is rectocele, and can be surgically repaired similarly with stitches. When the uterus drops due to weak support, a surgical repair can be done as well. This can be done without mesh and with or without a hysterectomy.
Often, prolapse symptoms can be improved with pelvic floor physical therapy or electomagnetic therapy (PF Toner). Patients can do Kegel pelvic floor exercises or have a physical therapist train them. There are a number of devices that can provide benefit including home use products with mobile apps.
PF Toner is a chair-like technology that uses electromagnetic energy waves to stimulate the muscles and nerves in the pelvic floor and build strength and improve sensation.
Another treatment for pelvic prolapse is a pessary device. This is a silicone ring or other form that is fitted inside the vaginal canal to support the vaginal walls and uterus like a brace. There are now self care devices similar to a pessary that patients can buy and use for vaginal support safely.
